When To Arrive For Your Appointment
Please arrive 15 minutes prior to your scheduled appointment to complete registration paperwork, if you are unable to print and complete the forms provided in advance. You can access our New Patient Form and Patient Form for Minors HERE.
Appointment Policy: If you need to cancel or reschedule your appointment, please notify us as early as possible so we may offer the time to another patient. Frequent cancellations, no-shows, or rescheduling may impact your ability to book future appointments with our office.
Bring The Following Items to Your Appointment
- Your current valid insurance card
- Referral or Authorization from your PCP, if necessary (this must be arranged with your PCP ahead of time)
- Your Specialist Co-pay
- Medical History, including all current medications and dosage information
- All previous medical records, x-rays. CT scans, MRI scans or other tests relating to your current problem
- If you have seen a physical Therapist bring the progress report if it is available
- If you have had a surgery with another physician please bring an operation report
Prescription Policy
Prescriptions cannot be dispensed by this office until a patient has been seen by Dr. Meisel or Dr. Carson. Prescription refills are handled daily from 8:00am to 4:30pm. Please have the name of your medication, dosage and pharmacy location available when requesting a refill. You should check with your pharmacy after 4:30pm to see if your refill is ready. Remember to check your medication requirements prior to the weekend and holidays.
